Client Forms

All forms must be completed prior to all procedures

In an effort to save time on the day of your procedure, please read all of the required client forms below.

Everyone is required to complete the Personality Quiz, Medical Form, and Consent Form. If you have had a prior procedure, the Release of Liability Form will also be required.

You will be signing these forms digitally after you arrive on the day of your procedure. A copy of the signed forms will be sent to your email on file.
